How It Works
In order for us to deliver the best care to you, you will need to follow the directions and provide the following information:
- Fill out the 'New Patient Medical History Form' which you can find if you scroll down on this page. When you fill out this form, make sure to include as much of the following items as you can:
- If you are seeking medical care then … State in one or two sentences, ‘IN YOUR OWN WORDS’ what the problem is.
- If you would like to have a question answered or something explained …Then state the question or what you would like to have explained.
- Upload your lab test results (blood tests; EKGs; stress tests Etc.) both recent and past onto the website.
- Upload any Radiology test images AND reports, both recent and past onto the website.
- State diagnoses obtained from previous Doctor’s visits.
- Provide list of treatments, including medications and doses.
